Background: Although convolutional neural networks (CNNs) achieve high diagnostic accuracy for detecting Alzheimer’s disease (AD) dementia based on magnetic resonance imaging (MRI) scans, they are not yet applied in clinical routine. One important reason for this is a lack of model comprehensibility. Recently developed visualization methods for deriving CNN relevance maps may help to fill this gap as they allow the visualization of key input image features that drive the decision of the model. We investigated whether models with higher accuracy (...) also rely more on discriminative brain regions predefined by prior knowledge. Methods: We trained a CNN for the detection of AD in N = 663 T1-weighted MRI scans of patients with dementia and amnestic mild cognitive impairment (MCI) and verified the accuracy of the models via cross-validation and in three independent samples including in total N = 1655 cases. We evaluated the association of relevance scores and hippocampus volume to validate the clinical utility of this approach. To improve model comprehensibility, we implemented an interactive visualization of 3D CNN relevance maps, thereby allowing intuitive model inspection. Results: Across the three independent datasets, group separation showed high accuracy for AD dementia versus controls (AUC ≥ 0.91) and moderate accuracy for amnestic MCI versus controls (AUC ≈ 0.74). Relevance maps indicated that hippocampal atrophy was considered the most informative factor for AD detection, with additional contributions from atrophy in other cortical and subcortical regions. Relevance scores within the hippocampus were highly correlated with hippocampal volumes (Pearson’s r ≈ −0.86, p < 0.001). Conclusion: The relevance maps highlighted atrophy in regions that we had hypothesized a priori. This strengthens the comprehensibility of the CNN models, which were trained in a purely data-driven manner based on the scans and diagnosis labels. The high hippocampus relevance scores as well as the high performance achieved in independent samples support the validity of the CNN models in the detection of AD-related MRI abnormalities. The presented data-driven and hypothesis-free CNN modeling approach might provide a useful tool to automatically derive discriminative features for complex diagnostic tasks where clear clinical criteria are still missing, for instance for the differential diagnosis between various types of dementia. The following article is a response to the position paper of the Hastings Center, “Ethical Challenges of Chronic Illness”, a product of their three year project on Ethics and Chronic Care. The authors of this paper, three prominent bioethicists, Daniel Callahan, Arthur Caplan, and Bruce Jennings, argue that there should be a different ethic for acute and chronic care. In pressing this distinction they provide philosophical grounds for limiting medical care for the elderly and chronically ill. We give a (...) critical survey of their position and reject it as well as any attempt to characterize the physician-patient relationship as a commercial contract. We emphasize, as central features of good medical practice, a commitment to be the patient's agent and a determination to acquire and be guided by knowledge. These commitments may sometimes conflict with efforts to have the physician serve as an instrument of social and economic policies limiting medical care. (shrink)

En tolkning af “Sein und Zeit” (herefter SuZ) kræver en afklaring af værensspørgsmålets rolle i værket, thi holdningen hertil bestemmer vurderingen af de mange enkeltanalyser, der indgår deri. Essayets tese er, at værensspørgsmålet er bærende i hele SuZ – hvilket ikke er så indlysende, som det måske synes. En række tolkninger har søgt at vise, at værensspørgsmålet er et pseudo-spørgsmål, der bør glemmes, men at de enkelte analyser i SuZ i sig selv er interessante . Andre tolkninger går ud fra, (...) at værensspørgsmålet selv aldrig bliver stillet i SuZ, men at der kun foretages en analyse af Dasein . Derfor bliver det et mål at vise, at de to hovedopgaver i SuZ, Daseinsanalysen og destruktionen af ontologiens historie, ikke er to hinanden uafhængige momenter, der hver især skal forberede grunden ud fra hvilken værensspørgsmålet skal stilles, men derimod er selve udfoldelsen af værensspørgsmålet. This book is less about disjunction than about the English word ‘or’, and it is less for than against formal logicians—more exactly, against those who maintain that formal logic can be applied in certain ways to the evaluation of reasoning formulated in ordinary English. Nevertheless, there are many things to interest such of those persons who are willing to overlook the frequent animadversions directed against their kind in the book, and this review will concentrate on them.

Translational research in medicine requires researchers to identify the steps to transfer basic scientific discoveries from laboratory benches to bedside decision-making, and eventually into clinical practice. On a parallel track, philosophical work in ethics has not been obliged to identify the steps to translate theoretical conclusions into adequate practice. The medical ethicist A. Cribb suggested some years ago that it is now time to debate ‘the business of translational’ in medical ethics. Despite the very interesting and useful perspective on the (...) field of medical ethics launched by Cribb, the debate is still missing. In this paper, I take up Cribb’s invitation and discuss further analytic distinctions needed to base an ethics aiming to translate between theory and practice. (shrink)

What is it to be a woman? What is it to be a man? We start by laying out desiderata for an analysis of 'woman' and 'man': descriptively, it should link these gender categories to sex biology without reducing them to sex biology, and politically, it should help us explain and combat traditional sexism while also allowing us to make sense of the activist view that gendering should be consensual. Using a Putnam-style 'Twin Earth' example, we argue that none of (...) the existing analyses in the feminist literature succeeds in meeting all of our desiderata. Finally, we propose a positive account that we believe can satisfy all the desiderata outlined. According to our theory, the genders 'woman' and 'man' are individuated not by their contemporary connections to sex biology, but by their historical continuity with classes that were originally closely connected to sex biology. Open peer commentary on the article “Second-Order Science: Logic, Strategies, Methods” by Stuart A. Umpleby. Upshot: Umpleby’s target article is important for bridging the gap between the natural and social sciences. While I agree with his claims, his proposals may not reach far enough. Concrete methods of empirical research, which are of crucial importance for a breakthrough, deserve further elaboration.

A person experiences and functions in two worlds – the external material world, and the internal mental world – whose objects differ in their nature, constitution, and reality. The 20th century scientists confess that hitherto the sciences were examining with their sophisticated apparatuses merely the observable externalappearances of phenomena, and that behind these lie a sensorily unobservable universe and the observer oneself! Exploring the external world for truth, through experiment and experience, contemporary scientists at their quest’s farthest end confront Consciousness (...) – the ineffable transcendental greatest mystery/Reality. The paper explains how while modern science is still grappling with Consciousness; the ancient philosophy of Advaita‐Vedanta has succeeded in identifying and isolating Consciousness – Brahman – through its psycho-physical-ethical spiritual praxis for “appropriating” it from the web of experience in the lived-world environing one and the all, to encapsulate the contemporary conclusions of the sciences (physics, biology, and psychology) in its holistic hypothesis which declares: Consciousness alone is real; the world is illusory; and oneself - no other than Consciousness/Brahman! Translational ethics (TE) has been developed into a specific approach, which revolves around the argument that strategies for bridging the theory‐practice gap in bioethics must themselves be justified on ethical terms. This version of TE incorporates normative, empirical and foundational ethics research and continues to develop through application and in the face of new ethical challenges. Here, I explore the idea that the academic field of bioethics has not yet sufficiently analysed its own philosophical foundation for how it can, and (...) should, be practically relevant; neither has it comprehensively discussed the limitations on what impacts bioethicists should pursue. As a result, there has not been adequate training on how to suitably and appropriately impact real‐world practices. Moreover, bioethical perspectives are often competing with other strong interests, for example, economic and political, which may weaken their impact on policy‐making. The TE approach I propose can not only facilitate practical impacts of academic bioethics by being better informed by real‐world ethical issues but it also supports targeted and ethical justifications of the actual impact of academic work in real‐world contexts. In this paper, I clarify the premises for this TE approach, identify further challenges and sketch out potential solutions for the implementation of this methodological framework. (shrink)
